790-X-2.16. Renewals.

(1) If for any reason a licensee fails to receive a renewal form, it shall be the
responsibility of the licensee to notify the Commission of this failure by
August 10 of the renewal period.
(2) Only the renewal of a license shall be effected with a renewal form. Any
changes constituting a transfer must be submitted in accordance with established
transfer procedures.
